Social Service
The Social Services Department is an integral part of the interdisciplinary team at SKKA A LIFE PLAN and provides support to individuals and their families, with the aim of better understanding their needs and responding to their requests in a more comprehensive manner.
Through the principles and methods of social work, the Social Services Department helps create a stable framework of support, information, and coordination that assists individuals and their families in finding their bearings, becoming empowered, and making the most of available resources and benefits.
The role of Social Services includes:
- the collection and documentation of the individual’s social history
- information about the office’s services, objectives, and support options
- ongoing collaboration with the individual and their family as part of comprehensive support
- raising awareness of and promoting social rights and benefits
- coordination with health and social services and other relevant agencies
- referral to appropriate services, where necessary, to better serve the individual and their family
- collaboration with the community, with the aim of increasing participation and access to available resources
